diaDexus and its affiliates will remain compliant with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) regarding all Protected Health Information as defined by this act collected or received by diaDexus or its affiliates. diaDexus or its affiliates will ask for written authorization before using or disclosing any protected health information for any purposes. This authorization will allow diaDexus and its affiliates to only disclose or use this protected health information for the sole purpose as described in the authorized consent. Authorization to diaDexus or its affiliates to use or disclose Protected Health Information can be revoked at any time through written communication to diaDexus. This revocation will not affect any use of disclosures made prior to submission of revocation but will prevent any future use or disclosures. Any Protected Health Information received by diaDexus or its affiliates will be used solely for purposes communicated to the patient prior to authorization of such disclosure. Any Protected Health Information received by diaDexus and any of its affiliates will remain confidential and will not be used for promotional purposes at any time. We may disclose Protected Health Information about you to your authorized personal representative, as defined by applicable law, or to an administrator, executor, or other authorized person responsible for your estate.
